Your
Loved One’s First Visit
Our staff will evaluate your loved one’s
wound and general health, and review their medical history.
A blood test may be involved, as may other special tests
that provide information about blood flow and oxygen flow
in the wound area.
The Healing Program
When the test results are in, we'll put together a treatment
program based on your loved one’s special needs.
This includes regular visits to the Wound Care Center to
provide treatment, evaluate progress, and make changes
as needed. Your loved one or their caregiver will be given
detailed instructions in home care, dressing (bandage)
changes, and protecting the wound from further injury.
Much of your loved one’s treatment success depends
on keeping appointments, following directions carefully,
and watching their progress closely between visits. Your
encouragement will help immensely. We ask that you help
them follow directions and keep appointments. Any time
you or your loved one has questions, our staff is available,
and we encourage you to talk with us.
How the Center Works with Your
Loved One’s Physician
Like any other medical specialist, the Wound Care Center
works with your loved one’s primary care physician.
We'll discuss their treatment program with their physician
and keep him or her updated, unless instructed otherwise.
Your loved one will continue to see their primary care
doctor for routine medical care.
What Your Loved One Should Bring to Their First Visit:
- Current List of
Medications
- Information about
their Medical History
- Patient Demographic
Sheet with Insurance Information
- If insured through
Medicaid, a Healthy Connection Referral Form (available
from your loved one’s primary care physician)
In addition, we will request this
information from your loved one’s primary care physician:
- Recent Progress Notes and/or Chart Notes
regarding the wound
- Recent Lab Results
- X-rays
- Diagnosis Reports
- Insurance
